Betsy Heimann made the shirt herself — "A lot of people have claimed responsibility for that shirt, but in fact it was a homemade shirt. We made it!" — and the famously short trousers were a budget fix: "Uma is very tall… I couldn't really find a pair of pants that fit her nicely that were long enough, and so I made an executive decision that since they were all too short I was going to make them really short."
Why we can't let it go
Mia Wallace is the best argument on this site that a great screen outfit does not require money. A white shirt, black trousers, a bob. Heimann built the shirt in-house and turned a fitting problem into the single most copied trouser length in film.
What makes it work is the contrast between how simple it is and how much attitude it carries. There's no jewellery, no bag, no colour, no shoes for most of the scene. It's basically a uniform.
And it's still the easiest Halloween costume in existence, which says something about how legible good costume design is. Two garments and a haircut, and everybody knows.

Get the shirt right and everything else follows: white, cropped or tied at the waist, with the top buttons open. It should look slightly too small rather than crisp.
The trousers are the detail people miss. Black, high-waisted, narrow, ending well above the ankle — cigarette length. Full-length black trousers make it a generic outfit.
No colour anywhere. No bag, no jewellery beyond maybe a thin watch. The whole thing depends on there being nothing else to look at.
And if you want it as a costume, the fringe does more than any garment. A blunt fringe and a black bob, and the reference lands before anyone sees the clothes.
